2010 Respecting Choices® POLST Paradigm Program
Advance Care Planning Facilitator Course

The Physician Order for Life-Sustaining Treatment (POLST) Paradigm system is gaining widespread national attention as a best practice for ensuring that end-of-life treatment preferences are honored. The POLST Paradigm Program (which includes West Virginia’s Physician Orders for Scope of Treatment [POST] program) provides a method for converting a person’s treatment preferences into medical orders that are recognized throughout the health care continuum (home care, long-term care, hospital, hospice, and during transport.)

We are asking all interested persons to complete the attached application. Attendees will be chosen from those that submit a completed application. A more detailed description of the course is attached. This course is limited to 30 participants at each site.
