The Center for Health Ethics and Law is the distribution point for the Physician Orders for Scope of Treatment (POST) form. This document is designed to help health care professionals know and honor the treatment wishes of their patients. It puts the advance directive into action by translating the patient’s treatment wishes into a medical order, centralizing information, facilitating record keeping, and ensuring transfer of appropriate information among health care professionals and across care settings.
